چکیده

The patient was a 72-yr-old male with chronic respiratory failure from COPD, successfully treated with nocturnal bilevel pressure NIV for 20 months. He was hospitalised in our respiratory intensive care unit (CHU de Rouen, Hôpital de Bois-Guillaume, Rouen, France) for ARF (pH57.26; arterial carbon dioxide tension (Pa,CO2) 11.4 kPa; arterial oxygen tension (Pa,O2)/ inspiratory oxygen fraction (FI,O2) 338). The patient presented with bilateral wheezing, encephalopathy and gaseous abdominal distension. Standard medical treatment associated with NIV failed to improve the patient’s condition, despite different settings with pressure-preset and flow-preset ventilatory modes, and careful management with facial and nasal masks. Five hours after admission, the patient’s condition worsened (pH57.12; Pa,CO2 19.2 kPa; Pa,O2/FI,O2 115). Intubation was then performed, subsequently revealing a laryngeal vestibule carcinoma, partially obstructing the glottis aperture. After tumoural laser resection, the patient was successfully weaned, extubated and then discharged home with his previous NIV treatment.

Early Effectiveness of Noninvasive Positive Pressure Ventilation on Right Ventricular Function in Chronic Obstructive Pulmonary Disease Subjects with Acute Hypercapnic Respiratory Failure

Introduction: Noninvasive positive pressure ventilation (NIPPV) has become an integral tool in the management of acute hypercapnic respiratory failure (AHRF) in chronic obstructive pulmonary disease (COPD). This study was performed to evaluate the early effects of NIPPV on pulmonary artery pressure (PAP), serum N-terminal pro BNP (NT-proBNP), and ventilatory parameters in the COPD patients with...

Efficiency and outcome of non-invasive versus invasive positive pressure ventilation therapy in respiratory failure due to chronic obstructive pulmonary disease

Background: Application noninvasive ventilation in the patients with exacerbation of chronic obstructive pulmonary disease (COPD) reduced mortality. This case-control study was designed to compare efficiency and outcome of non-invasive (NIV) versus invasive positive pressure ventilation (IPPV) in respiratory failure due to COPD Methods: The patients were assigned to NIV or IPPV intermittantl...

Noninvasive ventilation to shorten the duration of mechanical ventilation.

Noninvasive ventilation (NIV) successfully treats primary respiratory failure in chronic obstructive pulmonary disease (COPD), acute pulmonary edema, and, in some patients, hypoxemic respiratory failure. Increasingly clinicians have applied NIV in an effort to shorten the duration of mechanical ventilation by facilitating weaning and preventing or treating post-extubation respiratory failure. R...

Dependency on mask ventilation after acute respiratory failure in the intermediate care unit.

The predictive factors for long-term dependency (LTD) on noninvasive ventilation (NIV) immediately after acute hypercapnic respiratory failure (AHRF) have not been identified. The present authors studied 42 patients with chronic obstructive pulmonary disease (COPD) and 58 non-COPD patients successfully treated by NIV for an AHRF episode. Parameters at stable state, at admission for AHRF and dur...

Series 0contributions from the European Respiratory Monographs

Noninvasive ventilation and obstructive lung diseases. A. Cuvelier, J-F. Muir. #ERS Journals Ltd 2001. ABSTRACT: The key role of noninvasive positive pressure ventilation (NPPV) is well documented in chronic obstructive pulmonary disease (COPD) patients with acute respiratory failure (ARF) since it may avoid endotrachal intubation in w50% of cases when used as the initial treatment. However, cu...

Noninvasive positive pressure ventilation for respiratory failure caused by exacerbations of chronic obstructive pulmonary disease: a standard of care?

This editorial comments on a meta-analysis of the use of noninvasive positive pressure ventilation to treat patients with acute respiratory failure caused by chronic obstructive pulmonary disease (COPD) exacerbations published in the British Medical Journal earlier this year. Based on its analysis of seven randomized controlled trials that met pre-specified criteria, the meta-analysis demonstra...
